An Advance Directive is a document that tells your doctors and family what kind of care you want if you become too sick to talk or make decisions. Your family will have to make some decisions for you if you do not have an Advance Directive, and keep in mind that sometimes families do not agree about what a loved-one would want when that loved-one is very sick. Those are deeply painful situations for families. Writing down your wishes before a crisis is a gift to your family.

At Glenwood Healthcare, Inc., we want to know your wishes when it comes to medical issues. That's why we encourage you to fill out an Advanced Directives form, so that we can provide the care you want in times of medical emergency or severe illness.
